Part 1 lesson covers the 5-Why model, Design and Application Review method and Ishikawa (Fishbone) diagram to conduct an analysis. With those three RCA models in mind, please review and answer questions 1-2 in eRCA 1_Step 3 Part 1 assignment attached.
For question #3 — identify an event that triggers an RCA or a recurring failure to address. Select 5-Why, Design and Application Review or Ishikawa to practice investigating the problem. Submit proof of your work and conclusions from your investigation for this assignment. If you select 5-Whys as your investigation method, use the 5 Whys Activity template (downloadable resource in Step 3, Part 1 lesson) to submit your assignment.
